Request A List
If you are a Student, Faculty, or Staff member, and would like to have a list created for the purpose of an official campus club or organization, please fill out and submit the form below. Include a detailed reason for needing the list. Your request will be sent to an administrator, who will judge whether or not you meet the criteria needed for us to create a list for you. We will then contact you via phone or email. Our normal business hours are Monday through Friday 8:30 am - 4:30 pm.
